This section of the Medical Home Portal provides primary care physicians , care coordinators, and other health care professionals with information, tools, and resources to enable ongoing improvement in:
  • Care and outcomes for patients
  • Providing patient- and family-centered care
  • Realizing appropriate compensation for the extended or additional services involved in providing a comprehensive Medical Home
The Patient-Centered Medical Home provides an overview of what a medical home means to providers and families. About Children and Youth with Special Health Care Needs (CYSHCN)discusses the overriding care issues facing children and youth with special health care needs (CYSHCN). Further details are provided in the Care Coordinationsection pages about how to provide care coordination for those who could benefit most from it, how to code for these related services, and where to find training materials that will help guide efforts in improving the “medical homeness” of your practice.
Care Coordination in Portal Partner States offers information about specific care coordination activities and supports in the various states that partner with the Portal (see Partnering with the Portal).
Education & Schools provides information to help you understand how educators, schools, and school systems can help your patients and how you can help schools in assuring appropriate services are provided to your patients. You will find additional information and resources that might be of interest in the School Types and Options section of For Parents & Families.
Feeding & Nutrition addresses some of the causes of undernourishment, which is common in many chronic conditions and genetic syndromes. It provides evaluation and assessment tips for clinicians, detailed information about formulas, and information about calcium and vitamin D recommendations.
Health Supervision and Clinical Practice Guidelines contains published guidelines for chronic conditions in children; many of the conditions have corresponding diagnosis modules that can be found at Diagnoses & Conditions.
Medical Technology refers to devices and modifications that replace or support physiologic function. Each technology review in this section includes information about medical indications for its use, examples of medical orders to prescribe the technology and effectively communicate with home health care, and common questions and concerns.
Screening & Prevention offers guidelines and resources for screening for developmental, sensory, nutritional, and other health-related problems to allow early intervention for or prevention of chronic conditions and their complications.
Transition Issues provides information, tools, and resources to assist clinicians in making the numerous transitions CYSCHN and other children face go more smoothly.
