Residential facilities for older adults and/or people with disabilities who are unable to function in an independent living environment because they need assistance with toileting, bathing, dressing, medication management and administration, meals and housekeeping and other activities of daily living, but do not require nursing care on a regular basis. Living options range from institutions to settings that enable individuals with disabilities to live in their own homes or apartments with supportive services from community-based supported living providers. Youth ages 15 1/2 to 20. *Youth enter this program through the Southern Nevada Youth Coordinated Entry System.

Services Provided: Provides confidentially located furnished condominium units for unaccompanied youth. Youth are assigned a case manager and given assistance with food, clothing, and school. Guidance through transitioning to adulthood and gaining self-sufficiency. Designed for youth without a stable place to live. <em>**During COVID-19, to receive a housing assessment, please come to the NPHY Drop-In Center: walk-in's are welcome from 9am - 12pm and appointments are preferred 1pm - 4pm.​</em>
