Lyon County Human Services-Dayton Office-SOAR (Social Security Outreach Access and Recovery)

Program provides assistance and case management services to individuals who are defined as homeless or at risk of homeless and have a serious mental health illness, physical illness that affects the ability to work. Services include applying, monitoring, and being the authorized representative for the social security application process from beginning to end. Other services include community referral assistance, appointment scheduling assistance, transportation assistance support, interactive case management, volunteer opportunity supports, housing and income stabilization supports and referrals.
