Individuals whose disability causes a substantial functional limitation and lacks other resources.

Services Provided

Program can help individuals identify their Independent Living goals and the appropriate Assistive Technology (AT) that is necessary for the individual to care for themselves or receive care in their homes and their community. Also has resources to provide a variety of AT when no other resources are possible. Assistance in: developing independent living goals, identifying assistive technology and options, and information/referrals/technical information. Provides assistive technology needed for daily living home and community access; including home and vehicle modifications, durable medical equipment, visual aids, mobility devices, and personal communication technology when no other resources are available.
