Description: Programs that provide rehabilitation and/or habilitation services. Rehabilitation services involve a combination of treatment and education services which are designed to restore maximum functioning, a sense of wellbeing and a personally satisfying level of independence for individuals who have temporary or permanent disabilities. Habilitation services involve a combination of treatment and education services which are designed to either increase or maintain the physical, intellectual, emotional, and social functioning of individuals who have not reached age-appropriate developmental milestones. Services Provided: Provides day treatment in the form of habitation to individuals who suffer with acquired brain injury or traumatic brain injury. Provided as full day (six hours per day) or half day (minimum of three hours per day) and are provided five days a week or more. Service may occur in the recipients home, impatient setting who provided HBHS services, outpatient settings or in other community based settings.
