CHIPRA Funding Opportunity
The Idaho Department of Health and Welfare, Division of Medicaid, Children’s Healthcare Collaboration is offering funding for up to 12 months of support for a statewide collaboration to focus on pediatric patient centered medical home, evidence based quality improvement and care coordination.
This funding opportunity is open to any CHIPRA demonstration site. Demonstration sites are limited to Developmental Pediatrics, Pocatello Children’s Clinic and Primary Health Pediatrics.
The level of funding being offered is at a maximum of $28,500 for up to twelve months.
The Children’s Healthcare Improvement Collaboration is soliciting an application for funding for improving children’s healthcare in the State of Idaho. This solicitation will allow for better health outcomes for all children, with a particular emphasis on children and youth with special healthcare needs. The intent of this opportunity is to enable a demonstration site to improve coordination of care while transforming to a pediatric patient centered medical home. Applicants will be asked to provide sustainability plans for end of grant period funding.
