Helps individuals navigate their health system by helping with medical appointments, medical diagnosis, understanding tests, helping with medications, assisting with health insurance (Medicare, Medicaid, Medicare Advantage, and other insurances). Helps with moving patients to rehabilitation and/or hospice care. <br />
